Release of Liability

READ CAREFULLY – THIS AFFECTS YOUR LEGAL RIGHTS

In exchange for participation in the activity of The LYFE Foundation Live Career Conference organized by The LYFE Foundation, Inc., and/or use of the property, facilities and services of The LYFE Foundation, Inc., I agree for myself and (if applicable) for the members of my family, to the following:

  1.  AGREEMENT TO FOLLOW DIRECTIONS. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by The LYFE Foundation, Inc., or the employees, representatives or agents of The LYFE Foundation, Inc..
  2.  ASSUMPTION OF THE RISKS AND RELEASE. I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge The LYFE Foundation, Inc. for injury, loss or damage arising out of my or my family’s use of or presence upon the facilities of The LYFE Foundation, Inc., whether caused by the fault of myself, my family, The LYFE Foundation, Inc. or other third parties.
  3.  INDEMNIFICATION. I agree to indemnify and defend The LYFE Foundation, Inc. against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family’s use of or presence upon the facilities of The LYFE Foundation, Inc..
  4. FEES. I agree to pay for all damages to the facilities of The LYFE Foundation, Inc. caused by any negligent, reckless, or willful actions by me or my family.
  5. CONSENT. I, PARENT/GUARDIAN of minor attending The LYFE Foundation Live Career Conference, consent to the participation of my child in the activity of The LYFE Foundation Live Career Conference, and agree on behalf of the above minor to all of the terms and conditions of this Agreement. By signing this Release of Liability, I represent that I have legal authority over and custody of the minor.
  6. APPLICABLE LAW. Any legal or equitable claim that may arise from participation in the above shall be resolved under Arizona law.
  7.  NO DURESS. I agree and acknowledge that I am under no pressure or duress to sign this Agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this Agreement if I so desire.
  8.  ENFORCEABILITY. The invalidity or unenforceability of any provision of this Agreement, whether standing alone or as applied to a particular occurrence or circumstance, shall not affect the validity or enforceability of any other provision of this Agreement or of any other applications of such provision, as the case may be, and such invalid or unenforceable provision shall be deemed not to be a part of this Agreement.

I HAVE READ THIS DOCUMENT AND UNDERSTAND IT. I FURTHER UNDERSTAND THAT BY SIGNING THIS RELEASE, I VOLUNTARILY SURRENDER CERTAIN LEGAL RIGHTS.

No Refund/Cancellation/Postponement Policy for Registrations

Once paid, registration fees for The LYFE Foundation Live Career Conference and other events are non-refundable.

Sale or transfer of The LYFE Foundation Live Career Conference tickets or credits is not permitted.

We reserve the right to cancel or postpone The LYFE Foundation Live Career Conference or related event(s) in our sole discretion. If we cancel or postpone a Seminar or event, we will use reasonable efforts to reschedule the cancelled or postponed seminar or event to within twelve (12) months from its original start date. Registration fees paid by registrants for a cancelled or postponed seminar shall be applicable to the rescheduled seminar on a dollar for dollar basis only. In the event The LYFE Foundation, Inc. elects not to reschedule the cancelled Seminar or event, The LYFE Foundation, Inc. will make every effort to offer the registrant a refund. If this occurs, our responsibility is limited to a refund of any registration fee(s) already paid. The LYFE Foundation, Inc. is not responsible for airline tickets, hotels costs, other tickets or payments, or any similar fee penalties or related or unrelated losses, costs and/or expenses registrant may incur or have incurred as a result of any trip cancellations or changes.

The LYFE Foundation Live Career Conference dates and locations are subject to change without prior notice. The LYFE Foundation, Inc. reserves the right to change any and all instructors for each conference or event without notice.

By registering for a Seminar, you acknowledge that The LYFE Foundation, Inc. does not make any representation, warranty, or guarantee as to whether you will ultimately attain sufficient understanding and knowledge of any particular career.

Regardless of whether you attend or become a professional in a particular career, once paid, the registration fee becomes non-refundable. Any right to a refund is not dependent on whether you become a professional in a career represented at the conference.

Photograph & Video Release

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration.  I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.  I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. 

Photographic, audio or video recordings may be used for the following purposes:

By accepting this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet or in the public educational setting.

There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of The LYFE Foundation Live Career Conference.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational purposes.

If this release is obtained on behalf of a child under the age of 18, then my signature applies for myself and any child attending The LYFE Foundation Live Career Conference for whom I am a parent or legal guardian.
